The following major work elements are required as part of the Contractor's schedule and defined in the contract document, but is not limited to the following:
1. Pier reconstruction work for BIN 1075589 by zones. Pier work is restricted to four piers at a time, fascia to fascia.
a. Erect shoring
b. Demolish piers
c. Construct new piers
d. Install bearings
e. Restore the work area except for final paving (Item 4)
f. Proposed date each zone will be completed

2. Deck replacement activity for BIN 1075589 & BIN 107758E
a. Demolition of existing deck
b. New deck construction
c. Steel repairs
d. Proposed date deck replacement work in the vicinity of Ramp G (Stage 2c) will be completed, including paving portions of Ramp G
e. Proposed date deck replacement work at Ramp E and portions of BIN 1075589 Spans 55 & 56 (Stage 3c) will be completed, including paving portions of Ramp E/F
f. Proposed date bridge deck will be restored to its original lane configuration (completion of deck replacement Stage 8)

3. Utility work
a. ITS replacement
b. Underdeck lighting replacement

4. Miscellaneous work
a. Bridge painting
b. Parking area restoration (milling, paving, striping)
c. Proposed date that the parking area will be fully operational.

Additional required services include, but are not limited to the following:
Aggregates, Armored Joints/Joint Sealing, Asbestos Abatement, Asphalt Paving, Line Striping, Clearing & Grubbing, Concrete Ready-Mix, Curbs & Sidewalks, Demolition, Diamond Grinding, Drainage & Watermain, Electrical/Lighting/ITS/Tolling, Erosion Control, Fence, Railings, Frames, Grates, Signs, Sawcutting & Grooving, Hazardous Waste, Impact Attenuator, Survey, Inspection & Testing, Landscaping, Milling, Structural Steel Erection, Paint & Lead Abatement, MPT, Pest Control, Photography, Pipe Jacking, Cleaning, & Inspection, Precast Concrete Barrier & Deck, Bearings, Rebar, SIP/Shear Studs, Track Coating, Trucking, Containers & Dumpsters, Disposal & Waste Management, Vibration Monitoring, Waterproofing,

To be compliance with the contract requirements for NYS Civil Rights reporting, it is recommended that ALL DBE & NON-DBE Subcontractors and ALL DBE Vendors have an active EBO account prior to the bid date. Subcontractors/Vendors can access an EBO application, as well as Login & Password information using the following link: https://www.dot.ny.gov/dotapp/ebo/instructions

All proposals must include: Bid Item Numbers; Addenda's, Copy of EBO Registration, and Copy of NYSDOT DBE Certificate*
*All NYSDOT DBE certificates must be current at bid time and remain current for the duration of the project.

Plans, Specifications, Addendum, and Boiler Plate Templates of our Subcontract/Purchase Order ("Agreement") are available on SmartBidNet.com, your firm will be required to execute IF AWARDED the contract. You must base your bid upon the contractual terms and conditions as they are reflected in the Agreement, without amendment. The upstream contractual environment on the Project is demanding and we will not entertain revision to the contractual boilerplate, which should instead be reflected in your bid price. Please provide a price proposal to Furnish/Install or Supply your scope or supply of work including but not limited to work and jobsite delivery; all in accordance with and conforming to the project contract documents and additional requirements necessary to complete the work as specified herein. The referenced work scope shall also be performed to comply with the requirements of the project schedule, Milestones, including work staging and phasing, work restrictions and constraints, site access conditions and be coordinated with all other aspects of the project.
