The Rockland County Department of Mental Health (DMH) is seeking proposals from qualified non-profit agencies to
provide Transition Management (TM) and Re-entry (RE) services for individuals with Serious Mental Illness (SMI) and/or
substance use disorders (SUD) who are being released from the Rockland County Jail (RCJ). The purpose of this initiative
is to improve behavioral health outcomes, support successful community reintegration and reduce recidivism through
coordinated, person-centered post-release services.
